PPO
If you have PPO insurance then you may schedule directly with the GRID Clinic. We encourage you to involve your doctor.
HMO
If you have HMO insurance then you will need a referral from your primary doctor. If you provide us with your doctor's name and address we will forward program literature to them.
MEDICARE
Medicare (without an intermediary) is likely to cover your comprehensive evaluation and some of your ongoing care if you are obese (BMI >30) and you suffer from diabetes, hypertension, or hypercholesterolemia. Please contact the GRID Clinic for an evaluation.
MEDICAL
MediCal does not cover weight loss programs.
INJURED WORKERS
If you are a California injured worker and obesity is interfering with your treatment or recovery then you are entitled to reasonable treatment. Please provide us with your name and the name of your primary Workers Compensation treating doctor. We will provide your doctor with the necessary information to obtain authorization and referral.
PRIVATE PAY
Self-pay at the GRID Clinic is very affordable. Your comprehensive evaluation and first month of treatment may be as little as $285. Ongoing costs of treatment may be as little as $35 per month.
